Build an Anki-ready, recall-forcing study set from a messy page of lecture notes

I take decent lecture notes but turning them into flashcards is the boring step I always skip, so I cram and retain nothing.

Create a prompt that takes {{notes}} and a {{subject}} and outputs a clean set of question/answer flashcards optimized for active recall: one idea per card, questions that force retrieval (not yes/no), answers kept short, and a few 'why/how' cards that go beyond definitions. It should tag each card with a difficulty (easy/medium/hard) so I can schedule reviews, and group cards by topic.

Never invent facts that aren't in my notes; when the notes are thin on a topic, the prompt says so instead of filling the gap with plausible-sounding filler.
